Do we always have to use the tut.TOM or can we name our own files?

There's no need to always use the TUT.WAD name. Read the section of the manual called "Creating Your Own Project," (p.64- "Changing a TR4 File Name") it details the necessary steps for changing file names. It's a little more complex than you might expect, so be sure to make back-ups of your files before changing any names.

Can't emphasize enough: When you output a WAD, it MUST BE THE SAME NAME AS THE WAD YOU LOADED INTO YOUR PROJECT. If you load an existing project, such as karnak.prj, when you output the WAD you must save it as karnak.tom which creates the karnak.tr4 file in your DATA folder when you use the level converter. If you create your own project and load the catacomb.was, then you must stick with catacomb.tom when you output the WAD. If you want to call the .tr4 something different, you are better off renaming the entire WAD, but remember, you have to make these changes in the script or you won't be able to play your level. If your tr4 file has a different name from those in the script, you will have to change the entries in the script. If you do that, make sure you select the right block of script (the one that corresponds to the WAD you are using). This is all explained in the manual. Make sure you read the 'readme' as it contains an update pertaining to this procedure.

Roger that big man, just wanted to see if it could be done. A lot of people would prefer to create their own names instead of doing backups all the time.

When you create your own project and load in the WAD you have selected, say Catacomb.was, and you output your WAD as Catacomb.tom, it will overwrite the existing file. This is no big deal as long as you have the project file that created that particular TOM file becasue you can always remake it and the TR4 file. So, you can create your own project without changing any names or anything in the script, as long as you don't mind having to create the other files again if you need/want them or you can prevent having to redo them by renaming or putting in a safe folder before outputting the WAD and making the TR4 file of your own project.
