There are two ways:
If the file you downloaded ends with .tr4, it goes into the following folder:
Program Files\Core Design\TRLE\Data
For Example, Coastal.tr4 will go into that folder replacing a file of the same name, BE SURE TO FIRST make a backup of the ORIGINAL Coastal.tr4 (COPY and PASTE to the same folder)
Start the game from the
Start Menu\Programs\Core Design\Tomb Raider Level Editor\Setup Tomb Raider OR Run Tomb Raider

If the file you downloaded ends with .TOM, it goes into the following folder:
Program Files\Core Design\TRLE\Graphics\Wads folder first!!
For Example, Coastal.TOM will go into that folder replacing a file of the same name, BE SURE TO FIRST make a backup of the ORIGINAL Coastal.TOM (COPY and PASTE to the same folder)
Start the game from the
Start\Programs\Core Design\Tomb Raider Level Editor\Tools\Level Converter
Click Add under the Edit Script button
Click the .TOM file you want to convert
Click Open
Click Build All
When finished, close the entire window
Now Start the game from the
Start Menu\Programs\Core Design\Tomb Raider Level Editor\Setup Tomb Raider OR Run Tomb Raider

How to play the custom level you just installed:
This applies to both method
After starting the game, go to the New Game Screen
If the file you downloaded was named Tut1.Tr4 or Tut1.TOM, choose the Playable Tutorial Level game
If the file you downloaded was named City.Tr4/TOM, Coastal Ruins.Tr4/TOM, Catacomb.Tr4/TOM, Cleopatra.Tr4/TOM, etc, choose the game of the Same Title under the New Game list

Some of the Fan Created levels available for download in a Zip format may include the following files (But not all the time):
English.Dat
load.Bmp
Strings.h
Script.Dat
These files go into
Program Files\Core Design\TRLE
BE SURE TO FIRST make a backup of the ORIGINAL English.Dat, load.Bmp, Strings.h, Script.Dat(COPY and PASTE to the same folder)

Finally, if you downloaded and installed a Custom Made level named Coastal.TR4 for example and you downloaded another New Level in the meantime of the Same Name, installing the New Level will overwrite the Custom Made level of the same name that you've been playing.
Therefore, don't install the New Level of the Same Name until you are have finished the first one.
I keep track of all the Custom Made Levels by saving all the ZIP files containing the different levels in one folder on my desktop so I can switch back and forth between levels of the same name anytime I wish.
It may sound complicated but after a few times, you will get used to it.
