What OCB would I put on a puzzle piece that is on a 2 click high pedestal?

You would need to put "4" in the OCB.

The reference is on page 83.

Yes I do have the manual but I had Acrobat Reader version 3 so just about all the reference pages were blank. I just downloaded version 5 so the problem is fixed.
